Facial Membership Program Stipulations

1. Membership Term

    

   • To terminate your membership, you must provide notice at least 10 days before the next billing date. Cancellations received after this 10-day window will take effect the following month..

   • Billing is the first of every month and will be prorated accordingly

    

2. Monthly Service Inclusions

    

   • Members receive 1 customized facial per month.

   • Services do not roll over unless otherwise specified in writing.

    

3. Rollover Policy

    

   • You may roll over one missed facial per 6-month period.

   • Additional missed appointments are forfeited.

    

4. Appointment Scheduling

    

   • Members are responsible for scheduling their monthly facial.

   • A 24-hour cancellation policy applies. No-shows or late cancellations may forfeit that month’s service.

    

5. Payments

    

   • Monthly payments are auto-billed on the 1st of each month.

   • A one-time $75 enrollment/setup fee may apply.

    

6. Discounts & Perks

7. Sharing/Transferring Memberships

    

   • Memberships are non-transferable and may not be shared.

    

8. Pausing Membership

    

   • You may pause your membership for up to 1 month with written notice. Notice must be given within the 10 days of billing.

   • Extended pauses may require re-enrollment.

    

9. Early Cancellation

    

   • If canceling before the 3-month minimum, a $75 early termination fee applies.

   • Exceptions may be granted for relocation or medical reasons with proper documentation.

    

10. Membership Termination

• Visage’ MEDISPA reserves the right to terminate memberships due to policy violations, missed payments, or inappropriate conduct.
